Job Description

Position Summary:
30-hour Social Worker needed for the Boston Children’s Hospital Primary Care Center in Weymouth

This Primary Care Clinical Social Worker will be responsible for:
  • Providing clinical social work services to assist patients and families with personal, family, medical and environmental difficulties.
  • Performing psychosocial and risk assessments, assigning diagnoses and developing and implementing interventions and treatment plans.
  • Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan
  • Implementing and following up on treatment and provision of needed services.
Minimum Qualifications:
Education:
  • Master’s Degree in Social Work
Experience:
  • Flexibility and ability to work in fast-paced multidisciplinary team environment as well as excellent communication skills.
  • Experience with social services agencies, understanding of social determinants of health and trauma ideal.
  • Bilingual/bicultural/diverse candidates are encouraged to apply. Spanish speaking a plus!
Certification/Licensure:
  • MA LICSW required

The posted pay range is Boston Children’s reasonable and good-faith expectation for this pay at the time of posting.
